What to expect from oral corticosteroid treatment for bullous pemphigoid

Bullous pemphigoid is an autoimmune blistering condition. Oral corticosteroids reduce the immune attack on the skin, which can help new blisters and itch settle. Existing blisters still need time to dry and heal, and fragile skin may need dressings or additional treatment. Once control improves, the clinician may reduce the dose gradually or add another medicine to limit steroid exposure.

The treatment plan is individualized, particularly for older adults and people with diabetes, high blood pressure, osteoporosis, eye disease, or a history of infection. Improvement in new blister activity is usually assessed alongside wound healing and itch.

Take each dose as directed and follow the taper exactly. Do not stop suddenly after ongoing treatment. Keep blisters clean, avoid breaking them unless your clinician has shown you how, and use the recommended dressings. Attend reviews for skin examination, blood pressure, blood sugar, weight, mood, sleep, eye symptoms, and infection checks.

Get urgent help for breathing difficulty, facial swelling, confusion, severe weakness, black stools, or a serious eye symptom. These signs can reflect infection, treatment effects, or involvement needing immediate assessment.

Ask how you will know that new blister formation is controlled, how the taper will work, which dressings are suitable, and whether a steroid-sparing medicine is appropriate. Ask about bone, eye, blood pressure, and infection monitoring during treatment.
