What nutrition support can include

Cancer cachexia involves changes in appetite, metabolism, muscle and energy that are not always corrected by simply eating more. A dietitian may review weight changes, muscle strength, swallowing, nausea, pain, constipation, diarrhoea, taste changes and how much food or fluid feels manageable. The plan may use small frequent meals, nourishing drinks, preferred foods or texture changes.

Support can also include treatment of symptoms that make eating difficult and discussion of medicines or artificial nutrition when these fit the person’s condition and goals. The focus may be maintaining strength, supporting treatment, improving comfort or making meals less stressful rather than pursuing a target weight.

Practical steps at home

Offer food when appetite is strongest and keep portions small. Choose calorie- and protein-rich options that the person enjoys, unless the clinical team has given a restriction. Keep drinks available and ask about mouth care, nausea control and constipation treatment. Avoid pressure, arguments or guilt around eating; appetite can be affected by the illness itself.

Bring a record of foods and drinks tolerated, symptoms after meals and changes in strength to appointments. Ask for help from a dietitian, speech and swallowing specialist or palliative-care team when eating becomes tiring, unsafe or distressing.

When to seek medical help

Contact the care team promptly for inability to keep fluids down, repeated choking or coughing with meals, new confusion, very little urine, severe weakness, persistent vomiting or rapidly worsening symptoms. Urgent assessment is needed for severe breathing difficulty, collapse or signs of serious dehydration. Do not start tube feeding or supplements without discussing the person’s needs and goals.

Ask what the nutrition goal is now, which symptoms are limiting intake, whether supplements are suitable, how hydration should be managed, when swallowing assessment is needed and how the plan will change if eating becomes burdensome.
