What to expect from nail surgery for nail separation

Onycholysis is lifting of the nail plate away from the nail bed. Surgery is considered when loose nail is painful, repeatedly catching, severely damaged, or needs closer examination. The clinician first checks the nail and surrounding skin, then numbs the area before removing the selected nail material. The aim is to clear unstable tissue and make wound care easier; it does not instantly restore the nail’s appearance.

Tell the clinician about medicines, allergies, diabetes, circulation problems, and any previous reaction to local anaesthetic. Keep the dressing clean and dry as instructed, protect the fingertip or toe from knocks, and avoid picking at the healing area. Use only the recommended pain relief and attend the planned review. A replacement nail grows from the nail root, so visible improvement follows growth rather than the operation itself.

Contact the treating clinic if bleeding does not settle with firm pressure, the dressing becomes soaked, or pain is worsening instead of easing. Prompt assessment is also needed for spreading redness, increasing warmth, pus, fever, numbness, or a finger or toe that becomes unusually pale or blue.

Ask which part of the nail will be removed, how the nail bed will be protected, when the dressing should change, and what signs would suggest infection or a problem with regrowth.
