What to expect from nail surgery for nail fungus

Nail fungus, also called onychomycosis or fungal nail infection, can make a nail thick, crumbly, discoloured, or painful in footwear. Surgery may be considered when a nail is severely damaged, repeatedly catches, or needs removal so the underlying skin can be treated. Removing the visible nail does not by itself clear fungus from the nail unit or surrounding skin, so the clinician may discuss testing and topical or oral treatment. Other causes of nail change can look similar and may need assessment.

Tell the clinician about medicines, allergies, diabetes, circulation problems, and previous nail treatment. The digit is numbed before the nail or affected section is removed. Keep the dressing clean and dry as instructed, wear roomy footwear, and avoid picking at the healing area. Use only the antifungal treatment recommended for you, and keep feet dry by changing damp socks and allowing shoes to air. Attend follow-up so the new nail and infection plan can be reviewed.

Contact the clinic for worsening pain, spreading redness, heat, swelling, or drainage. Fever, red streaking, persistent bleeding, or a wound that opens needs prompt medical advice. If the nail remains painful, becomes newly discoloured, or shows signs of infection during regrowth, arrange a review rather than restarting treatment without guidance.

Ask whether the nail change has been confirmed as fungus, why surgery is being considered, and whether part or all of the nail will be removed. Discuss testing, antifungal options, dressing changes, footwear, expected regrowth, and how recurrence will be monitored.
