What the assessment covers
A clinician may ask about sadness, loss of interest, energy, appetite, sleep, concentration, guilt, hopelessness, irritability, and physical symptoms. They may ask when changes started, whether they come and go, and how they affect work, study, relationships, and self-care.
You will usually be asked directly about thoughts of death, suicide, or self-harm. This is a safety check, not an accusation. The clinician may also ask about past episodes, family history, medicines, alcohol or drugs, trauma, and periods of unusually high energy or little need for sleep, because these details influence treatment choices.
How to prepare and take part
Before the visit, note symptoms, sleep, medicines, recent stressors, and any change in appetite or functioning. Be honest about thoughts that feel difficult to say aloud. You may bring someone you trust, ask for an interpreter, or request a quieter pace.
The plan could involve talking therapy, medicine, practical support, a physical health review, or a combination. Agree how progress will be checked and what to do if symptoms worsen. If medication is discussed, ask about expected effects, side effects, and review arrangements.
When to seek urgent help
Seek immediate help if you may act on thoughts of suicide, have a plan, cannot stay safe, or are at risk of harming someone else. Contact emergency services or go to an emergency department when danger is immediate. Urgent assessment is also needed for severe confusion, hallucinations, or an extreme change in sleep and energy.
