What to expect from iron therapy for anaemia in pregnancy

Short answer
Iron therapy can improve low iron and haemoglobin during pregnancy, but treatment usually needs regular use and follow-up blood tests.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Treatment forms
- Oral iron or an intravenous infusion may be used, depending on your situation.
- Common effects
- Nausea, constipation, stomach discomfort, and darker stools can occur with oral iron.
- Follow-up
- Repeat blood tests help check whether treatment is working.
What treatment may involve
Your maternity clinician may recommend iron tablets or liquid iron when blood tests show anaemia linked with low iron. The chosen preparation and dose depend on your results, pregnancy stage, symptoms, other medicines, and whether you can absorb or tolerate oral iron. If oral treatment is unsuitable or does not improve the blood count, an intravenous iron infusion may be considered.
Iron does not correct anaemia instantly. Energy and breathlessness may change gradually, while repeat blood tests show whether haemoglobin and iron stores are responding. Even after you feel better, continue treatment for the length advised so your iron stores can be replenished.
Taking iron in pregnancy
Take iron exactly as prescribed and do not double a missed dose unless your clinician tells you to. Oral iron can cause nausea, constipation, stomach discomfort, or darker stools. Taking it with food may make it easier to tolerate, although some products work best away from meals; follow the instructions for your specific product. Keep iron away from children, and tell your clinician about antacids, calcium supplements, thyroid medicine, or other treatments because timing can affect absorption.
If side effects make treatment difficult, contact your maternity team rather than stopping without advice. They may adjust the preparation, timing, or route of treatment.
When to seek care
Contact your maternity care team promptly if you cannot keep the medicine down, have severe or persistent abdominal pain, develop troublesome constipation, or notice symptoms are worsening despite treatment. Seek urgent medical help for severe breathlessness at rest, chest pain, fainting, confusion, or heavy vaginal bleeding. During an infusion, report rash, swelling, wheezing, dizziness, or sudden feeling unwell immediately to the staff caring for you.
Questions for your doctor or midwife
Ask which type of iron is being prescribed, when to take it alongside your prenatal vitamins, and what to do if a dose is missed. Clarify when your blood count will be checked, what improvement the team expects to see, and how long treatment should continue. If you have had stomach problems with iron before, ask whether another preparation or an infusion is appropriate.

Improvement is often gradual. Your symptoms and follow-up blood tests help show whether treatment is helping.
What if iron tablets upset my stomach?
Tell your maternity clinician. They can review the product and timing, suggest ways to manage side effects, or discuss another route when appropriate.
