What to expect from imiquimod treatment for genital warts

Imiquimod is a cream that helps stimulate a local immune response against genital warts. It is applied to the affected external skin according to the schedule your clinician gives you. The cream is generally left on for the directed time and then washed off. Treatment can take several weeks, and improvement may appear gradually rather than immediately.

Redness, swelling, soreness, itching, scaling, or small areas of raw skin can occur where the cream is applied. These effects can signal local inflammation, but severe discomfort or extensive skin breakdown needs clinical advice. Imiquimod does not protect against every sexually transmitted infection, and treated warts can return.

Wash your hands before and after application, use only the prescribed amount, and keep the cream away from the eyes, mouth, vagina, and inside of the anus unless specifically directed. Do not apply it to broken skin. Avoid sexual contact while the cream is on the skin; it may irritate a partner and can weaken condoms or other barrier methods.

Keep scheduled reviews so the clinician can check the response and adjust treatment if irritation becomes difficult. Do not add wart removers, steroid creams, or other products to the area without asking.

Contact your clinician promptly for severe pain, marked swelling, spreading redness, pus, fever, difficulty passing urine, or bleeding that does not settle. Stop and seek advice if the treated area becomes deeply ulcerated or the reaction prevents normal activity. Urgent care is appropriate for facial or eye exposure, breathing symptoms, or a rapidly worsening allergic-type reaction.

Ask which lesions should receive cream, how long to leave it on, what to do after a missed application, and when sexual contact is safe. Also ask how irritation will be managed, when the first review should occur, and what other treatment is suitable if the warts persist.
