What to expect from hand therapy for rheumatoid arthritis

Short answer

Expect therapy to be adjusted to your inflammation, with gentle movement, joint protection, activity advice and support for useful hand function.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main aim
Protect joints while preserving useful movement
Plan changes
Exercise may be gentler during inflammation

What the assessment covers

The therapist considers swelling, tenderness, range of motion, grip, fatigue and how rheumatoid arthritis affects tasks such as dressing, typing or opening containers. During an active flare, the emphasis may be on comfort, gentle motion and protecting irritated joints rather than strengthening. A splint can sometimes rest or support a joint, but fit and wearing time need individual guidance. Hand therapy works alongside medical treatment; it does not replace medicines used to control inflammation.

Using your plan safely

Follow the specific exercise dose you are given and report increased swelling or symptoms that remain worse after practice. Alternate heavier and lighter tasks, use both hands for load sharing, and keep frequently used items easy to reach. Your therapist may teach pacing, ergonomic changes, and ways to reduce force during gripping or pinching. Reviews allow the programme to change as inflammation settles or returns, so keep a record of tasks that are difficult.

When to contact your care team

Tell your rheumatology or therapy team about a flare that is not settling, rapidly increasing swelling, new weakness, or a splint that causes pressure, colour change or numbness. Seek urgent help for severe sudden pain after an injury, a cold or blue hand, or loss of movement that develops quickly. These changes need assessment rather than extra exercise.

Useful questions

Ask how inflammation changes your exercises, which activities need joint protection, how long to wear a splint, and whom to contact during a flare.
