What to expect from fracture fixation surgery for an ankle fracture

An ankle fracture can involve one or more bones and may disturb the joint surface or ankle alignment. During fixation surgery, the surgeon repositions the broken parts and holds them with implants such as plates, screws or other stabilising devices. The exact approach depends on the fracture pattern and the condition of the soft tissues.

After the operation, a dressing, splint or boot protects the ankle. Swelling, bruising and stiffness are common early features. Follow-up reviews check the wound, alignment and healing before weight bearing or exercise is increased.

Keep the leg elevated when resting and follow instructions about ice, dressings and bathing. Use crutches or another aid exactly as taught, and put weight through the leg only within the limit prescribed. Keep the incision protected and do not remove a splint or boot unless instructed.

Move the toes and other joints only as advised. Arrange a safe route at home, keep essential items within reach and attend wound and imaging reviews. Physiotherapy can later address ankle movement, balance, strength and walking.

Contact the surgical team for increasing wound redness, drainage, fever, worsening pain, calf swelling or new numbness. Seek urgent help for sudden breathlessness or chest pain, or if the foot becomes cold, blue, severely painful or difficult to move.

Ask how much weight is allowed, when the splint or boot may change, how to protect the wound, when driving is safe, and which milestones guide a return to work or sport.
