What to expect from fertility preservation for childhood cancer

Some cancer treatments may affect future fertility, depending on the medicines, radiation, surgery, treatment area, and the child’s stage of development. The oncology team may involve a paediatric fertility specialist before treatment starts. Options differ for children who have not entered puberty and for adolescents who have. They can include preserving reproductive tissue or, when development allows, collecting and freezing sperm, eggs, or embryos. The team should explain what is known, what is uncertain, and how stored material would be managed later.

Ask for a fertility discussion as soon as cancer treatment is planned. Bring questions about timing, anaesthesia, discomfort, privacy, and how the child’s wishes will be included. Tell the team about developmental stage, medical conditions, medicines, and family concerns. A counsellor or psychologist may help the child understand choices without pressure. The preservation plan must remain coordinated with treatment, so cancer care should not be changed without the oncology team.

Call the treating team for fever, increasing pain, significant bleeding, vomiting that prevents fluids, faintness, or a wound that becomes red or drains. Seek urgent care for severe breathing trouble or sudden serious symptoms. After treatment, report delayed puberty, absent or changing periods, testicular changes, or other concerns about development so the oncology and fertility teams can review them.

Ask how the planned cancer treatment may affect fertility, which options are realistic at this age, and whether a procedure could delay treatment. Discuss consent and assent, storage arrangements, future access, and follow-up testing. Ask who will explain the plan to your child and where emotional support is available.
