What to expect from emollient therapy for ichthyosis

Ichthyosis causes ongoing dryness and visible scaling because the outer layer of skin does not shed and retain moisture in the usual way. An emollient coats the surface, fills tiny gaps between skin cells and helps water stay in the skin. You may notice less tightness and softer scale before the appearance changes. The effect fades when applications stop, so this is generally a maintenance approach rather than a one-time treatment.

Some products are thicker than others. Ointments tend to feel greasy but can be useful for very dry areas, while creams may feel easier during the day. A clinician can help choose a formula that suits the type of ichthyosis, age and body area.

Apply the emollient generously after bathing or washing, while the skin is still slightly damp. Smooth it on rather than rubbing scaly skin vigorously. Reapply when the skin feels tight, and pay attention to elbows, knees, hands, legs and any area that cracks. Use a separate clean scoop or pump where provided to reduce contamination in a shared container.

Keep bathing gentle: use lukewarm water, avoid harsh fragranced cleansers and pat the skin dry. Do not pick scale or add exfoliating ingredients without advice, because irritated skin can sting and become more vulnerable. Emollients can make floors, steps and clothing slippery, so allow the product to settle and take care around movement.

Arrange a clinical review if the skin becomes increasingly red, painful, warm, swollen, cracked or weepy, or if crusting and fever develop. These changes can signal irritation or infection rather than simple dryness. Seek prompt advice if a product causes marked burning, a spreading rash or facial swelling. Review is also sensible when the current routine no longer controls scaling, or when a baby or child has difficulty regulating body temperature or staying hydrated.
