What to expect from dry needling for piriformis syndrome

Your physiotherapist checks your hip, back and leg symptoms and chooses a safe position, often lying on your side or stomach. The skin is cleaned before a fine sterile needle is placed in the muscle being assessed. You might notice a prick, pressure, twitch or temporary ache. Because the piriformis lies deep in the buttock, the clinician works carefully and avoids treating through numb, infected or irritated skin. The session may finish with mobility work or strengthening.

Tell the clinician about anticoagulants, bleeding problems, pregnancy, recent injections, altered sensation or an operation in the area. Wear clothing that permits access to the buttock and hip. Afterward, keep the area gently mobile and avoid strenuous running, lifting or prolonged stretching until discomfort settles. A small bruise or muscle ache can be normal for a short period. Dry needling is commonly used alongside exercises that address hip control, sitting tolerance and nerve-related irritability.

Tell your clinician if buttock pain, numbness or leg weakness becomes more intense or does not settle as expected. Seek urgent care for new loss of bladder or bowel control, numbness around the groin, rapidly worsening weakness, fever with a hot swollen site, or difficulty breathing.

Ask which muscles will be treated, how the clinician will protect nearby nerves, what activity is suitable afterward, and how dry needling fits with your exercise programme. Clarify what change should prompt a review of the diagnosis.
