What to expect from custom orthotics for limb length discrepancy

A clinician first considers whether the difference comes from bone length, joint position, or movement habits. The insert may include a lift or other shaping to alter how your foot meets the ground. It does not lengthen the shorter limb, and the amount of correction may be deliberately partial to help your body adapt.

The goal is a steadier stride and less strain on areas such as the foot, knee, hip, or lower back. Results vary because the insert addresses walking mechanics rather than every possible cause of discomfort.

Wear the orthotics for short periods at first, then increase use as your feet and legs adjust. Put them in shoes with enough depth and a firm heel counter; take them out if the shoe becomes tight. Mild pressure or awareness can occur during the adjustment period, but worsening pain should be reported.

Bring your usual walking shoes to the fitting and attend planned reviews. A clinician may change the lift, trim the device, or assess your gait after you have used it in daily activities.

Arrange a clinical review if pain steadily increases, you develop rubbing or a skin sore, or the orthotic makes you limp. Seek prompt care after a new injury with marked swelling, inability to bear weight, or sudden weakness or numbness.

Ask how the limb difference was measured, how much correction is planned, which shoes are suitable, and when the fit should be reassessed. Also ask which symptoms should settle during adaptation and which mean the device needs changing.
