What to expect from custom orthotics for hammer toe

A hammer toe bends at one or more toe joints, so the raised area may rub against the shoe while the ball of the foot takes extra load. An orthotic can include cushioning, a pressure-relieving area or support under the forefoot. The design depends on whether the toe is flexible, stiff, painful or rubbing. It will not reliably undo a fixed deformity.

Pair the device with a wide, deep toe box and introduce it for short periods. Check the top and tip of the toe after wearing it, as well as the ball of the foot. Keep the skin clean and inspect it if sensation is reduced. A fitting review can address crowding, sliding or pressure that has shifted to a neighbouring toe.

Seek a review for persistent rubbing, a corn, blister, skin break or increasing pain. Get prompt care if the toe becomes hot and swollen, changes colour, loses sensation, or you cannot walk after an injury. These problems may require examination rather than simply wearing the device longer.

Ask whether your toe can still be moved by hand, which shoe shape is needed, whether a pad or orthotic feature is appropriate, and how skin pressure will be monitored.
