What to expect from custom orthotics for bunions

A bunion, or hallux valgus, is a change in alignment at the base of the big toe that can create pressure in shoes. A custom device is made after examining your foot, movement and footwear. It may support the arch or guide pressure away from the irritated joint and prominent area. Orthotics cannot straighten an established bunion, but a better pressure pattern may make walking and selected shoes more comfortable.

Break them in gradually and inspect the skin around the bunion, arch and heel after wearing them. Choose shoes with a roomy toe box and enough depth; squeezing the toes can undermine the comfort goal. Report rubbing, redness that remains, or pressure that makes the big toe more painful. Your clinician may adjust the device, review your gait or add exercises for foot and lower-limb control.

Arrange a review if pain continues to build, the device causes a blister or numbness, or you cannot fit it without crowding the toes. Seek prompt assessment for sudden severe pain, marked swelling, skin breakdown, loss of feeling, or a hot red joint with fever, particularly if you have diabetes or reduced circulation.

Ask which pressure area the device will unload, whether your everyday shoes are suitable, how to protect the skin, and what signs mean the orthotics need modification.
