What to expect from custom orthotics for ankle osteoarthritis

Ankle osteoarthritis can make the joint stiff, painful and less tolerant of uneven ground. Custom orthotics are designed around your foot and may use cushioning, support or alignment changes to spread forces through the foot and ankle. The aim is more comfortable movement, not removal of the arthritic joint changes. Your clinician may combine orthotics with footwear advice, mobility work, strength exercises or other treatment.

Start with limited wear in your most supportive shoes and increase the time only when the device feels comfortable. Bring the orthotics and shoes to review appointments so fit and alignment can be checked together. Watch your skin, ankle swelling and walking pattern. Use handrails or other support on unfamiliar surfaces if stiffness affects balance, and follow the activity plan that keeps movement within a tolerable range.

Ask for a fitting review if you develop rubbing, pressure, numbness, new knee or hip pain, or worsening ankle symptoms. Seek prompt care after a fall, sudden inability to bear weight, major swelling, unusual warmth with fever, or rapidly increasing pain.

Ask what alignment or pressure change is planned, which shoes are suitable, how to judge a useful response, and what alternatives exist if the device limits comfort or balance.
