What to expect from a cortisone injection for rotator cuff tendinitis

Rotator cuff tendinitis can irritate the tendons involved in lifting and rotating the arm. Cortisone is delivered around the painful shoulder structures to calm inflammation. The aim is to reduce symptoms enough for useful movement; it does not strengthen the tendon or correct contributing factors such as poor load tolerance, stiffness or repetitive overhead strain.

A clinician may examine your neck, shoulder movement and strength before choosing the injection site. Some people notice improvement after the local anaesthetic wears off and the anti-inflammatory effect develops; others receive limited relief.

Before the appointment, mention diabetes, allergies, blood-thinning medicine and current skin or systemic infection. After the injection, protect the shoulder from strenuous activity for the period advised. Gentle movement can prevent guarding, but stop exercises that cause sharp or escalating pain.

Use the better symptom window to practise the prescribed mobility and strengthening work. Adjust desk, sleep and overhead tasks if they repeatedly provoke the shoulder. Keep follow-up if pain returns, because symptom relief alone does not show that the tendon is ready for full loading.

Get medical advice promptly for spreading redness, drainage, increasing warmth, fever or worsening pain at the injection site. Seek urgent help for sudden inability to lift the arm, new hand numbness with weakness, breathing difficulty, facial swelling or another severe allergic symptom.

Ask which structure is causing your pain, what activity limits apply after the injection, and which exercises should begin once soreness settles. Ask how long to try rehabilitation and what other options would be considered if symptoms return.
