What to Expect from a Cortisone Injection for IT Band Syndrome

How the structures involved in IT Band Syndrome differ from normal

Short answer

Expect a focused injection near the irritated outer knee, short-term tenderness, and a gradual return to activity guided by symptoms.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the injection means

IT band syndrome involves irritation where the iliotibial band passes the outside of the knee, often with repeated bending and straightening. Corticosteroid medicine may calm inflammation around the painful area when symptoms remain limiting. The injection does not lengthen the band or correct training, strength, footwear, or movement factors that may keep irritating the knee. A clinician may combine it with activity modification, hip and leg strengthening, flexibility work, and a gradual return plan.

Managing activity afterward

Tell the clinician about diabetes, blood-thinning medicine, allergies, skin infection, and previous injection reactions. Keep the knee away from strenuous running, hills, or repeated squats while the injection site is sore. Do not use early pain relief as a reason to restart full training immediately. Follow the exercise plan for hip control, leg strength, and running mechanics. Reintroduce distance and intensity in stages, stopping to review the plan if outer-knee pain returns. Note pain during stairs, walking, and exercise to make follow-up more useful.

When to seek care

Contact the treating service promptly if pain keeps increasing, the knee becomes hot and markedly swollen, or fever or feeling unwell develops. Seek urgent help for breathing difficulty, facial swelling, or widespread hives. New leg weakness, persistent numbness, inability to walk safely, or a foot that becomes unusually pale or cold needs prompt assessment. A brief injection-site ache can happen, but progressive symptoms deserve review.

Questions for your doctor

What structure will the injection target? Which running or gym movements should I pause? When can strengthening and sport-specific drills resume? How will we address the cause of repeated irritation if pain returns?

Questions people ask

Will the injection fix IT band syndrome by itself?

No. It may settle local inflammation, while exercise, training changes, and movement work address factors that continue to stress the area.

Can I train as soon as pain improves?

No. Return gradually and follow the clinician's guidance because early pain relief may not mean the tissues are ready for full loading.
