What to expect from a cortisone injection for biceps tendinitis

Biceps tendinitis affects the tendon at the front of the shoulder, often causing pain when lifting, reaching or rotating the arm. A clinician may use an examination and, when needed, imaging to choose the injection site. The medicine is placed near the irritated tissue; it does not rebuild a tendon or correct every cause of shoulder pain. Relief can develop over several days and may vary. A brief increase in soreness can occur after the injection.

Tell the clinician about diabetes, blood-thinning medicine, allergies, infection, previous shoulder injections and any planned surgery. Follow the advised limits on lifting and overhead work after the procedure. Keep the area clean, use a cold pack wrapped in cloth if advised, and return to comfortable movement as symptoms settle. Physiotherapy may then address shoulder mobility, rotator-cuff and shoulder-blade control, and a gradual return to loading. Do not use early pain relief as a reason to suddenly resume heavy exercise.

Contact the treating service promptly for increasing redness, warmth, swelling, drainage, fever, or pain that is severe or worsening rather than settling. Seek urgent help for new arm weakness, loss of sensation, marked swelling, breathing difficulty or signs of a serious allergic reaction. If shoulder pain or function does not improve as expected, arrange reassessment so other sources of pain, including a tendon tear, can be considered.
