What to expect from CO2 laser resurfacing for xanthelasma

Xanthelasma consists of yellowish deposits that commonly affect the inner eyelids. CO2 laser resurfacing treats the surface of selected deposits with concentrated energy. The clinician first checks the diagnosis, the thickness and position of the deposits, and the health of the surrounding eyelid skin. Eye shields or other protection are used because the treatment is close to the eye.

After treatment, the area may be red, swollen, tender, or lightly oozing before a crust forms. Healing time varies with treatment depth and the size of the treated area. Temporary lightening or darkening of the skin is possible, with greater concern when the skin is exposed to sun or becomes irritated.

Tell the clinician about eye disease, contact lens use, cold sores, medicines that affect bleeding or healing, and previous eyelid procedures. Arrive with clean skin if instructed. After treatment, use only the cleanser, ointment, and dressings recommended for the area. Do not scratch, peel, or scrub the crust, and avoid getting unapproved products into the eye.

Pause swimming, sauna use, intense heat, and eye makeup until the surface has closed and your clinician says they are suitable. Protect healed skin from sunlight. Xanthelasma may recur because laser treatment addresses visible deposits rather than every factor that can contribute to their formation; medical review may therefore be part of follow-up.

Call the clinic if swelling, pain, redness, or drainage is increasing, or if the wound develops thick discharge, spreading redness, fever, or a bad smell. Obtain urgent eye care for reduced vision, severe eye pain, marked light sensitivity, or swelling that prevents the eye from opening. Do not wait for a routine appointment when vision is affected.

Ask how the diagnosis was confirmed, whether the deposits are suitable for laser treatment, how the eyes will be protected, and what alternatives exist. Clarify expected crusting, pigment risk, wound care, makeup restrictions, follow-up timing, and how recurrence or deposits near the eyelid margin will be handled.
