How clinical Pilates is tailored
A physiotherapist may ask how your back pain behaves during sitting, walking, lifting and sleep. They can observe breathing, spinal movement, hip control and how you use your abdominal and back muscles. Exercises may begin with supported positions and simple breathing or pelvic movements, then progress as control improves. The session is not a test of flexibility. The goal is to help you move safely and steadily while connecting exercises with tasks that matter to you.
Making sessions useful
Wear clothes that allow easy movement and tell the physiotherapist about flare-ups, pregnancy, recent injury or other health conditions. During practice, describe whether a movement causes familiar discomfort, a new sharp pain or symptoms travelling into a leg. Use the home exercises at the agreed level and pause if symptoms escalate rather than adding repetitions on your own. Short movement breaks and gradual return to normal activities often support the exercise plan.
When to seek medical care
Arrange urgent medical assessment for new loss of bladder or bowel control, numbness around the inner thighs or genitals, marked leg weakness, inability to walk, fever with severe back pain or pain after major trauma. Contact your clinician promptly if pain becomes progressively worse, night symptoms are unusual for you or a new leg symptom persists. These features require assessment before continuing an exercise programme.
