What to Expect from Breast MRI for a BRCA Gene Mutation

A BRCA1 or BRCA2 mutation can alter a person’s breast screening plan. MRI can reveal areas that are difficult to assess on mammography or ultrasound, but an MRI finding is not a diagnosis by itself. If contrast is used, it travels through a vein and helps some tissue patterns stand out. The scan does not use ionising radiation. Images are reviewed by a radiologist, who may recommend comparison with earlier scans or another test.

Tell the MRI team about metal implants, a pacemaker, kidney problems, prior contrast reactions, pregnancy, breastfeeding and anxiety in enclosed spaces. You may need to remove jewellery and change clothes. You lie face down on a padded table with your breasts positioned in an opening, then the table moves into the scanner. The machine makes loud knocking sounds, so hearing protection is provided. Staying still matters; a staff member can communicate with you throughout.

Tell staff immediately if you feel severe discomfort, breathing difficulty, swelling, widespread itching or dizziness during or after contrast. Contact your care team for a delayed rash or worsening injection-site pain. Seek urgent help for trouble breathing, facial swelling or fainting.

Ask whether contrast is planned, how your mutation changes the screening schedule, whether you need to avoid food or drink, and when the report will be available. Ask how an unclear finding is handled and whether earlier images should be brought for comparison.
