What to expect from bone-strengthening therapy for metastatic breast cancer

When breast cancer has spread to bone, the normal balance between building and removing bone can be disrupted. Bone-strengthening medicines reduce excessive bone removal. This may help lower the chance of fractures and other bone complications, and may ease some cancer-related bone pain. The treatment does not take the place of hormone therapy, chemotherapy, targeted treatment, radiotherapy or pain relief when those are recommended.

Your oncology team will select the medicine and timing around your overall plan. Treatment may be an infusion or an injection. Blood tests can check kidney function and calcium, while a dental review may be advised before treatment if there are dental concerns or planned procedures.

Tell every clinician about the treatment before dental extraction or other invasive dental work. Follow instructions for calcium, vitamin D and fluids rather than choosing supplements yourself. Mention new pain, reduced movement or a change in walking early, so your team can assess the bone and consider imaging or other support. Ask about safe activity, lifting and fall prevention; staying gently active may be appropriate, but painful or high-impact movements may need adjustment.

Get urgent help for sudden severe bone pain, a suspected fracture, new weakness or numbness in the legs, or loss of bladder or bowel control. Contact your team quickly for jaw pain, swelling, loose teeth, or a sore in the mouth that does not heal. Confusion, severe thirst, repeated vomiting or much less urine can indicate a calcium or kidney problem and needs prompt assessment.

Ask which bone medicine fits your treatment plan, how often it will be given, and what monitoring is needed. Discuss dental clearance, calcium and vitamin D, pain control, safe exercise, and how bone treatment will be coordinated with radiotherapy or other systemic treatment.
