What to expect from a blood test for folate deficiency

Expect a small blood sample from your arm, with results interpreted alongside your symptoms and other blood counts. A clinician cleans the skin, inserts a needle, fills one or more tubes, and removes the needle. You may feel a brief sting or mild bruising afterwards. Ask whether you need to fast or pause any supplements; follow the instructions from the service arranging the test.

Testing commonly includes a full blood count and a folate measurement. The blood count can show changes such as large red blood cells or anaemia, while the folate result helps assess available folate. A result can be affected by recent supplements, diet, illness, or timing, so one result is not interpreted in isolation. Vitamin B12 may also be checked because the two deficiencies can produce similar blood-count changes, and treating folate deficiency alone can be unsuitable when B12 deficiency has not been considered.

Tell the clinician about vitamins, prescribed medicines, pregnancy or plans for pregnancy, alcohol use, and recent illness. Do not start high-dose folic acid before discussing the test if B12 deficiency is possible. Results may be reviewed with your diet, symptoms, blood count, and any reason your body may not be absorbing nutrients. If deficiency is confirmed, treatment and a repeat test may be recommended; the plan depends on the cause and your overall health.

Arrange prompt medical advice for worsening breathlessness, chest discomfort, fainting, marked weakness, or a racing heartbeat. Seek urgent help for sudden confusion, severe difficulty walking, or new weakness or numbness. These symptoms can have causes other than folate deficiency and should not wait for routine results.
