What to expect from bisphosphonate therapy for vertebral compression fracture

A vertebral compression fracture can occur when a weakened spinal bone collapses. Bisphosphonates slow bone breakdown and are used to reduce the risk of additional fragility fractures; they do not immediately rebuild the collapsed vertebra or remove pain on their own. Your plan may also include activity guidance, physiotherapy, posture work, pain relief, and an assessment for other causes of the fracture.

Use the tablet or infusion exactly as prescribed. If it is an oral medicine, follow directions about water, food, and staying upright to reduce irritation of the oesophagus. Keep follow-up appointments for kidney function, calcium and vitamin D review, pain progress, and fracture prevention. Avoid lifting or bending movements that your clinician has restricted, while gradually returning to safe activity under guidance.

Seek urgent care for new leg weakness, numbness around the groin, loss of bladder or bowel control, or severe rapidly worsening back pain. Contact your treatment team for swallowing pain, chest discomfort, severe cramps, new thigh or groin pain, or jaw pain after dental treatment. A fall with sudden new spinal pain needs prompt assessment.

Ask when treatment should begin, how it fits with fracture healing, and which movements are safe now. Discuss kidney and calcium checks, dental procedures, the dose schedule, missed doses, and how your clinician will review future fracture risk.
