What to expect from bisphosphonate therapy for Paget's disease of bone

Paget's disease causes areas of bone to remodel more actively than usual. The resulting bone may become enlarged, misshapen, or mechanically weak. Bisphosphonates reduce the activity of bone-resorbing cells and can settle active disease. Treatment may be considered for bone pain, active changes in a higher-risk location, or preparation for a procedure. It does not automatically correct an established deformity.

Your clinician may assess symptoms, blood tests related to bone activity, kidney function, calcium levels, and dental issues. Follow the instructions for your tablet or clinic infusion carefully. Maintain prescribed movement and physiotherapy, but adapt activities if a weight-bearing bone is painful or deformed. Keep monitoring visits because symptoms and bone activity do not always change at the same pace.

Report worsening bone pain, a new change in limb shape, hearing or balance changes, or difficulty walking. Seek urgent care for severe weakness, facial swelling, breathing trouble, fainting, or symptoms after an infusion that feel intense or rapidly worsening. Contact your clinician about swallowing pain, significant muscle cramps, or jaw pain that does not settle.

Ask whether your disease is currently active, what outcome the medicine is intended to achieve, and how activity will be tracked. Clarify the route and timing, calcium and vitamin D advice, kidney precautions, dental planning, and what symptoms should trigger an earlier review.
