What to expect from biologic therapy for nail psoriasis

Nail psoriasis can cause pits, ridges, discolouration, thickening, crumbling, or separation from the nail bed. A biologic medicine reduces a targeted part of the immune inflammation linked with psoriasis. It does not instantly repair the nail plate that has already formed.

Visible change usually follows nail growth, so the root and nail bed are assessed as well as the free edge. Fingernails and toenails grow at different speeds. A damaged nail may continue to look abnormal while healthier nail advances, and a fungal infection or injury can create similar changes.

Complete any recommended examination or nail sample before treatment, and tell your dermatologist about joint pain, skin psoriasis, infections, and current medicines. Follow the injection or infusion schedule precisely. Keep nails short, avoid picking under them, and protect hands from prolonged water or chemical exposure with suitable gloves.

Take occasional photographs in consistent lighting so changes are easier to compare at review. Do not stop treatment because the nail still looks abnormal early on. Your clinician may combine a biologic with local nail care or another psoriasis treatment.

Report fever, persistent cough, breathlessness, severe diarrhoea, or a spreading skin infection. Seek urgent help for facial swelling or breathing difficulty after a dose. Contact your dermatologist if a nail becomes acutely painful, swollen, or produces pus, since infection or a separate nail problem may need attention.

Ask how nail response will be measured, when photographs or examinations will be repeated, and whether testing is needed to exclude fungus. Ask what to do about a missed dose, planned procedure, infection, or new joint symptoms.
