What to expect from biologic therapy for hidradenitis suppurativa

Hidradenitis suppurativa causes recurring inflammation in areas such as the armpits, groin, buttocks, or under the breasts. A biologic medicine targets a specific part of the immune response involved in that inflammation. The aim is to reduce painful lumps, drainage, and the formation of new lesions; existing tunnels or scars may need separate care.

Response is assessed over time rather than after one dose. Some people notice fewer flares, while others need a different medicine or additional treatment. Your dermatologist may check the skin, symptoms, infection history, vaccinations, and other medicines before and during therapy.

Before starting, tell your clinician about recurring infections, tuberculosis exposure, hepatitis history, pregnancy plans, allergies, and medicines that affect the immune system. Screening tests may be needed. Follow the prescribed injection or infusion schedule and learn how to store and administer an injection if it is used at home.

Keep a simple record of painful areas, drainage, missed doses, and side effects. Do not squeeze lesions or apply irritating products to open skin. Dressings, pain relief, wound care, and surgery may still be part of the plan.

Contact your care team promptly for fever, a worsening cough, shortness of breath, persistent diarrhoea, painful urination, or a spreading warm skin area. Seek urgent help for severe allergic symptoms such as facial swelling or trouble breathing. A rapidly enlarging, very painful abscess may need drainage.

Ask which biologic is appropriate, what screening is required, how quickly response will be reviewed, and what to do if you miss a dose. Ask how open wounds, procedures, vaccines, pregnancy, or an infection should affect the schedule.
