What to expect from an allogeneic stem cell transplant for aplastic anaemia

Aplastic anaemia leaves the marrow unable to make enough blood cells. Before transplant, the team reviews blood counts, organ function, infections, medicines, and donor compatibility. Conditioning prepares the marrow environment and reduces the chance that the body rejects donor cells. The donor cells are infused through a central line, not surgically implanted. If they engraft, they begin making white cells, red cells, and platelets. Until that happens, you may need red-cell or platelet transfusions and medicines to prevent or treat infection.

Because donor immune cells can react against the body, the team gives immune-suppressing treatment and checks for graft-versus-host disease. Signs can involve the skin, mouth, bowel, liver, or lungs. The plan is adjusted according to symptoms, blood results, and how the graft is developing.

Plan for a protected recovery with a caregiver who can help with medicines, meals, transport, and communication with the unit. Wash hands carefully, follow food and visitor guidance, and avoid close contact with anyone who has an infection. Take every prescribed medicine on schedule; do not add herbal products or over-the-counter medicines without checking first. Expect tiredness and appetite changes while counts recover. Keep appointments for blood tests, transfusions, line care, immune-suppression review, and vaccination planning.

Call the transplant team urgently for fever or chills, new cough, breathing difficulty, severe headache, confusion, unusual bleeding, black stools, persistent vomiting or diarrhoea, or inability to keep fluids down. Report a new rash, painful mouth sores, yellow skin or eyes, reduced urine, or sudden weight gain. These symptoms can signal infection, bleeding, organ stress, or graft-versus-host disease and need prompt assessment.

Ask why transplant is recommended for your aplastic anaemia, what donor choices exist, whether conditioning is planned, how transfusions will be managed, and which medicines protect the graft. Ask when you can leave hospital, who to call outside clinic hours, and how activity, diet, dental care, contraception, fertility, and vaccinations will be handled during recovery.
