What to expect from an allogeneic stem cell transplant for acute myeloid leukaemia

For acute myeloid leukaemia, the team first checks disease status, organ health, infections, and donor matching. Conditioning treatment makes space in the bone marrow and helps control leukaemia cells. Donor stem cells are then given through a central line, much like a transfusion; there is no operation to place them in the bone. The cells travel to the marrow and gradually produce white cells, red cells, and platelets. During this period, blood counts can be very low, so transfusions, antimicrobial medicines, and protective precautions may be needed.

The donor immune cells may help recognise remaining leukaemia cells, but they can also attack healthy tissues. This is called graft-versus-host disease. Medicines are used to reduce this risk, and the team watches the skin, mouth, gut, liver, and lungs for changes.

Arrange practical help before admission: transport, meals, medicines, and support at home. Follow instructions about food safety, hand hygiene, visitors, and avoiding people who are unwell. Take medicines exactly as prescribed and keep a written list of temperature readings and symptoms. Recovery is gradual; fatigue, appetite changes, mouth soreness, and altered taste can occur. Keep follow-up appointments for blood tests, medication changes, line care, and disease monitoring. Do not restart work, strenuous exercise, or routine vaccinations until the transplant team advises you.

Contact the transplant team immediately for a raised temperature or chills, breathlessness, new cough, chest pain, confusion, severe weakness, persistent vomiting or diarrhoea, inability to drink, bleeding, or a new rash. Report yellowing of the eyes, dark urine, worsening abdominal pain, or rapidly increasing swelling. After discharge, use the team’s emergency number rather than waiting for the next appointment, because infection and graft-versus-host disease can worsen quickly when blood counts and immunity are recovering.
