What to Expect From ACL Reconstruction for an ACL Tear

Short answer
After ACL reconstruction for an ACL tear, expect staged knee rehabilitation focused on swelling control, movement, strength and stability.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the procedure changes
ACL reconstruction uses a graft to replace the torn anterior cruciate ligament and help control forward and twisting movement of the knee. The surgeon may also assess or treat related meniscus or cartilage injuries. Early symptoms can include pain, swelling, limited bending and difficulty activating the thigh muscles. Recovery is measured by function and examination, not simply by how the knee feels on one day.
What recovery involves
Use crutches, a brace, ice or prescribed medicine only as directed. Elevate the leg when resting and complete the exercises taught by your care team. Rehabilitation commonly progresses from reducing swelling and regaining knee straightening to improving bending, leg strength, balance, landing control and activity-specific movement. Do not return to cutting, pivoting or contact activity until your clinician has assessed readiness.
When to contact the care team
Seek prompt advice for increasing wound redness, drainage, fever, uncontrolled pain, new calf swelling or marked stiffness. Urgent care is needed for chest pain, severe breathlessness, fainting, or a foot that becomes cold, pale, numb or weak. A new giving-way episode, locking knee or sudden injury during recovery should also be reported.
Questions for your doctor
Ask which graft and additional procedures are planned, how much weight you may place on the leg, when driving or work may be possible, and which tests will guide return to sport.

Questions people ask
Is physiotherapy part of ACL reconstruction recovery?
Yes. Guided rehabilitation is used to restore movement, strength, coordination and knee confidence.
Can a knee feel better before it is ready for sport?
Yes. Symptoms may settle before strength and movement control have recovered, so clearance should follow clinical assessment.
