What Is the First Treatment for Patellar Tendon Rupture?

Short answer
The first treatment for a patellar tendon rupture is urgent assessment and protection, usually followed by tendon repair surgery and rehabilitation.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Priority
- Protect the knee and obtain urgent assessment
- Recovery focus
- Repair protection followed by graded rehabilitation
What a rupture involves
The patellar tendon helps connect the kneecap to the shin and allows the quadriceps to straighten the knee. A complete tear can interrupt this mechanism, causing sudden pain, swelling, a change in the kneecap’s position, and difficulty lifting the straight leg.
Until the injury is assessed, the knee is commonly kept supported and movement is limited. Examination and imaging help determine whether the tear is complete, where it is located, and whether other structures were injured.
What to do while waiting for care
Do not test the knee repeatedly or attempt forceful stretching. Keep weight off the leg unless a clinician has specifically allowed it, and use support during transfers. Arrange urgent orthopaedic review, because delay can make restoration of the tendon’s normal length and function more difficult.
If repair is advised, Post-Operative Rehabilitation is gradual. Early protection is followed by carefully increased knee motion, quadriceps activation, walking control, and strengthening. The exact pace depends on the repair and the surgeon’s instructions.
When to seek urgent care
Seek emergency assessment after a sudden knee injury if you cannot raise the straight leg, cannot bear weight, or notice major swelling or deformity. Numbness, a cold foot, or rapidly increasing pain also requires immediate attention.
Questions for your doctor
Is the tear complete? What protection is needed before treatment? What does Tendon Repair Surgery involve, and which activities will rehabilitation restrict at each stage?

Questions people ask
Can a complete patellar tendon rupture heal with exercises alone?
A complete tear may not restore the knee’s straightening mechanism with exercise alone, so an orthopaedic assessment is needed to discuss repair.
Why is early review important?
Early review identifies the tear and protects the knee while the treatment team plans how to restore alignment and function.
