What is the first treatment for Merkel cell carcinoma?

Merkel cell carcinoma is an uncommon skin cancer that can behave aggressively. The team first confirms the diagnosis from a biopsy and assesses the tumour, nearby lymph nodes, and any spread. Tumour removal surgery may be used to take out the visible cancer, sometimes with assessment or removal of nearby lymph nodes. Radiotherapy can be recommended after surgery or used when surgery is not suitable. Immunotherapy may be discussed when disease has spread or returned, or when the care team expects it to offer useful control.

Take your pathology report and scan results to the appointment if you have them. Ask whether the proposed operation includes a margin of healthy-looking tissue, whether lymph nodes need assessment, and how treatment could affect movement, sensation, or appearance. If radiotherapy or immunotherapy is suggested, ask what the aim is and how it fits with surgery. Keep a written list of medicines and new skin changes, and tell the team about immune conditions or medicines that suppress immunity.

Contact the cancer team promptly if a skin lesion grows quickly, bleeds repeatedly, becomes painful, or changes colour, or if a nearby lump appears. After treatment, urgent advice is appropriate for fever, worsening wound redness, heavy bleeding, breathing difficulty, or new weakness. New symptoms do not by themselves show that the cancer has spread, but they should be assessed without delay.
