What Is the First Treatment for Actinic Keratosis?

Actinic keratosis appears as a rough, scaly, or crusted patch on sun-exposed skin. Cryotherapy uses cold to destroy a selected lesion, and the area then heals as the treated skin sheds. This approach is commonly considered when there is a distinct lesion that can be examined clearly.

The starting treatment changes when there are many lesions, a broad sun-damaged area, or uncertainty about the diagnosis. A clinician may discuss topical 5-fluorouracil, imiquimod treatment, or photodynamic therapy. Skin cancer screening may be recommended when the examination shows additional areas needing assessment.

Do not scratch or pick a treated crust. Follow wound-care instructions, keep the area protected from the sun, and tell the clinician if the lesion has changed before treatment. Record new rough patches and arrange review rather than treating an unexplained mark with leftover creams. Follow-up after cancer treatment may be advised when a clinician has identified a higher-risk history or related skin problem.

Seek a skin examination for a lesion that grows, becomes thick, painful, tender, or bleeds, or for a sore that does not heal. Contact the treating clinic after cryotherapy if pain, swelling, redness, or discharge is worsening rather than settling. A lesion that looks different from nearby patches deserves individual assessment.

Ask whether the diagnosis is clear, why cryotherapy is suitable, and whether the lesion needs sampling. Ask if surrounding sun-damaged skin needs a field treatment such as imiquimod, topical 5-fluorouracil, or photodynamic therapy, and when the area should be reviewed.
