What is the first treatment for Achilles tendon rupture?

Short answer
The first treatment for an Achilles tendon rupture is prompt assessment followed by immobilisation in a position that protects the tendon.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why early protection matters
A rupture is a significant tear in the tendon connecting the calf muscles to the heel. The foot may feel weak, and pushing off or climbing stairs can become difficult. Examination and imaging, when needed, help determine the injury and guide the plan.
A walking boot or another form of immobilisation can limit ankle movement while the tendon ends are protected. Some people are treated without surgery; others may be offered Achilles tendon repair based on the tear, health, activity needs and clinical assessment.
What to do after the injury
Stop sport and avoid stretching the ankle or walking without the support prescribed for you. Keep weight off the leg if advised, and elevate it when resting. Do not change the boot position or remove its wedges unless your treating team instructs you.
Once the tendon is protected, rehabilitation is introduced in stages. Post-operative rehabilitation follows repair when surgery is chosen; non-operative care also requires a carefully timed progression of movement and strength.
When to seek care
Seek urgent assessment after a sudden snapping sensation, a new gap near the heel, rapid swelling, bruising or loss of push-off. Get prompt help if toes become cold or discoloured, numbness develops, or pain and tightness increase substantially inside the boot.
Questions for your doctor
Ask whether immobilisation or repair is appropriate, how much weight you may place on the leg, and when the boot angle will change. Clarify wound or skin checks, driving and work restrictions, and the milestones used to advance rehabilitation.

Avoid unprotected walking until you have been assessed and told how much weight is safe.
Is surgery always needed?
No. A clinician may discuss immobilisation without surgery or Achilles tendon repair after considering the injury and your circumstances.
