What if vaginal oestrogen therapy does not relieve vaginal dryness?

A clinician can check how the product is being used, whether enough time has been allowed for benefit, and whether another problem is causing the dryness or irritation.

Vaginal oestrogen acts locally, so it may improve hormone-related changes without addressing every source of discomfort. Scented washes, bubble baths, douching, some lubricants, and friction can irritate delicate tissue and make symptoms persist.

Dryness may also occur alongside soreness, itching, unusual discharge, skin changes, urinary symptoms, or pain with penetration. These features can point to a need for examination rather than a simple change in dose.

Use the treatment exactly as prescribed and keep a note of when you apply it, which symptoms remain, and whether symptoms follow sex, washing, or urination. Do not use extra applications to try to speed up relief.

Avoid perfumed products around the vulva and vagina. A plain, water-based lubricant can reduce friction during sex; a regular vaginal moisturiser may help with day-to-day comfort. Ask a pharmacist or clinician which products fit your treatment and health history.

Book a follow-up if symptoms have not improved, are returning, or are affecting sleep, intimacy, movement, or urination. The clinician may review the formulation and application method, examine the area, and consider non-hormonal options or treatment for a different cause.

Contact a clinician promptly for bleeding after sex, bleeding after menopause, new sores or blisters, marked swelling, severe or worsening pain, fever, or discharge with a strong or unusual smell. Seek urgent help for heavy bleeding, severe pain, fainting, or feeling acutely unwell.

Do not assume persistent dryness is only a treatment issue when these symptoms are present. Assessment can identify irritation, infection, skin disease, or another condition that needs different care.

Ask whether your symptoms fit the expected effect of the product, whether the application technique is correct, and when a review should happen. Mention any bleeding, discharge, urinary changes, pain with sex, or products used on the area.
