What if topical antifungal treatment does not relieve scaly skin?

Scaling is a skin response rather than a diagnosis. It may occur with fungus, eczema, psoriasis, dryness, contact dermatitis, seborrhoeic dermatitis, or inflammation after an injury. Antifungal treatment cannot correct every cause, and the scale may remain while an irritated skin barrier recovers. The site matters: scalp, face, hands, feet, and skin folds have different common triggers and treatment needs.

A clinician may examine the border, colour, thickness, cracks, blisters, nails, and nearby skin. Mention whether the area burns or itches, whether it is spreading, and which products have touched it.

Use the antifungal exactly as directed unless a clinician tells you to stop, and avoid adding acids, scrubs, steroid creams, or multiple remedies. Wash with a gentle cleanser, use a plain moisturiser on dry skin when appropriate, and avoid picking or peeling the scale. Keep a short record of where the scale appears and how it changes after each application. Bring the product to your appointment so the clinician can check its active ingredient and directions.

Seek prompt assessment if scaling is accompanied by rapidly spreading redness, warmth, swelling, pus, severe pain, fever, or extensive skin peeling. Get urgent help for swelling of the lips or face, breathing difficulty, or a widespread rash after applying a product. Do not delay review when the scalp, eyes, genitals, or large areas of broken skin are involved.
