What if thyroid hormone replacement does not relieve unexplained weight gain?

Thyroid treatment may need checking if hormone levels are not in the intended range or doses are missed. However, weight can also change with fluid retention, appetite, activity, sleep, stress, mood, other medicines, and conditions affecting metabolism. Thyroid replacement is intended to correct hormone deficiency; it is not a general weight-loss medicine. A pattern over time is more useful than a single reading, particularly if swelling, breathlessness, bowel changes, or appetite changes are present.

Keep taking the prescribed dose and do not add extra thyroid medicine or weight-loss products without advice. Record when the change began, appetite, activity, sleep, menstrual changes if relevant, swelling, and all medicines or supplements. Your clinician may check thyroid blood tests, review how the tablet is taken, and consider other explanations. Aim for steady meals and manageable movement rather than severe restriction. If food, body image, or weight concerns are causing distress, say so directly; support can be part of the plan.

Seek urgent help for sudden breathlessness, chest pain, fainting, severe weakness, confusion, or rapidly increasing swelling. Arrange a prompt appointment for rapid weight change, new swelling of the legs or face, persistent vomiting, marked thirst or urination, or a major change in bowel habits. These symptoms may need assessment beyond adjusting thyroid treatment.
