What if TENS therapy does not relieve lower back pain?

TENS changes how electrical signals are delivered through the skin, but it may not address the source of back pain. Lack of benefit can relate to electrode placement, settings, the activity that triggers symptoms, muscle or joint strain, nerve irritation, or a condition needing a different approach.

Record when pain appears, whether it travels into a leg, and what happens with walking, sitting, or bending. Have a clinician check pad placement and settings rather than increasing intensity independently. Seek urgent medical care for new loss of bladder or bowel control, numbness around the groin, severe or progressive leg weakness, or sudden inability to walk. Prompt review is also needed with fever, unexplained weight loss, or back pain after significant trauma.

Ask whether the pain pattern suggests nerve involvement, whether TENS remains worth using, which activities should be paced, and what change would trigger imaging or specialist review.
