What If Scabies Treatment Does Not Relieve Itchy Skin?

Scabies treatment kills mites, but the immune reaction in the skin can take time to settle. Itchy skin may also persist when the product causes irritation, when treatment did not cover the required areas, or when close contacts were not managed as advised. New burrows, fresh spots, or symptoms in another household member raise concern for ongoing transmission. Widespread itch without typical scabies signs can have another cause, such as eczema, an allergy, or a medicine reaction.

Write down the treatment name, application date, areas covered, and any missed instructions. Tell the clinician whether the itch is improving, staying the same, or accompanied by new lesions. Use a plain moisturiser if suitable, avoid hot showers and fragranced products, and keep nails short to limit skin breaks. Follow advice about washing or isolating clothing and bedding, and do not repeat scabies medicine unless a clinician recommends it. A clinician may need to inspect the skin and revise the diagnosis or plan.

Seek prompt assessment for pus, honey-coloured crusts, increasing warmth, fever, painful sores, or rapidly spreading redness, which can signal infection after scratching. Get urgent help for trouble breathing, swelling of the lips or face, faintness, or widespread blistering after treatment. Arrange earlier review for a baby, an older person, reduced immunity, or a thick crusted rash.
