What If Scabies Treatment Does Not Relieve Itching Worse at Night?

Itching can persist after mites have been treated because the skin may continue reacting to mite debris. Ongoing symptoms can also occur when the medicine was not applied to every advised area, was washed off too soon, or close contacts were not assessed and treated when advised. New burrows, fresh bumps, or itching that is spreading suggest that active scabies or another diagnosis needs review. Itching by itself cannot confirm that treatment has failed.

Tell the clinician when treatment was applied, which product was used, how long it stayed on the skin, and whether any doses or applications were missed. Mention children, pregnancy, immune problems, and medicines because these can affect the plan. Follow the prescribed instructions for close contacts and clothing or bedding. Avoid repeatedly applying treatment without advice, since this can irritate already inflamed skin. A soothing moisturiser and keeping nails short may reduce scratching while the cause is clarified.

Arrange prompt review for new burrows, spreading rash, pus, crusting, fever, or painful cracks, as scratching can lead to a bacterial skin infection. Seek urgent care for facial swelling, breathing difficulty, or widespread blistering after a medicine. Infants, older adults, people with reduced immunity, and anyone with a widespread crusted rash should be assessed promptly because scabies may behave differently in these groups.
