Short answer

If narrowband UVB phototherapy does not relieve scaly skin, arrange a review to reassess the diagnosis, skin barrier, treatment plan, and possible irritants.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Remember
Scale can have several causes, and light therapy does not treat every cause.
Before review
Avoid picking, abrasive exfoliation, and unsupervised changes to UV exposure.

Reasons scaling may persist

Scaling is a surface sign rather than a diagnosis. It can reflect inflammation, dryness, a fungal process, irritation, allergy, or another skin condition. Narrowband UVB may help some inflammatory disorders but may not address every cause of scale. The skin can also remain dry or flaky while deeper inflammation settles. Treatment response should be judged with the appearance, symptoms, distribution, and examination together.

What to do before the appointment

Photograph the affected areas in consistent light and note whether scale is fine or thick, whether it cracks or bleeds, and whether itch or pain accompanies it. List all creams, cleansers, hair products, and medicines used on the skin. Apply the moisturiser recommended by your clinician, avoid picking and harsh scrubs, and do not alter UVB exposure or use tanning equipment to chase a result. The clinician may revise the diagnosis, check for infection or irritation, and change treatment.

When prompt assessment is needed

Seek prompt care for rapidly spreading scale, extensive redness, painful cracks, pus, fever, blistering, or skin that becomes very tender. Urgent help is needed for facial swelling with breathing difficulty or a widespread blistering rash. These changes can require assessment for infection, severe inflammation, or a treatment reaction.

Questions to ask

Ask what diagnosis best fits the pattern and whether a scraping, swab, or other examination is needed. Ask whether your skin barrier is too irritated for the current schedule, which moisturiser or cleanser to use, when to reassess, and which symptoms mean phototherapy should be paused.

Questions people ask

Is persistent scaling proof that narrowband UVB is unsuitable?

No. The clinician may need to clarify the cause of the scale, assess irritation, or combine or change treatments.

Can moisturiser affect phototherapy?

Some products can alter skin irritation or light sensitivity, so use the product and timing recommended by your phototherapy team.
