Short answer

If muscle relaxant medication does not relieve back muscle spasm, contact a clinician to reassess the cause, medicine plan, movement limits and need for additional treatment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Medication
Do not change the dose without advice.
Movement
Use gentle activity and change position regularly.

Why the spasm may continue

A muscle relaxant may reduce tightening without removing the reason the back is guarding. A strained muscle, irritated spinal joint, disc-related pain, poor lifting mechanics or prolonged sitting can keep symptoms active. Pain can also be coming from a nerve or another area rather than from muscle alone.

The medicine may be unsuitable, taken at the wrong time, or limited by side effects that prevent a useful dose. Sleepiness and dizziness can occur with some medicines, so do not drive or mix them with alcohol unless a clinician has specifically confirmed it is safe. Never increase the dose on your own.

Useful next steps

Ask the prescriber to review the diagnosis, dose, other medicines and treatment duration. Describe what movements provoke the spasm, whether pain travels into a leg, and whether numbness or weakness is present. Keep gently active if you can, changing position regularly rather than staying in bed for long periods. Heat may feel soothing for some people; protect the skin and stop if it worsens symptoms. A tailored physiotherapy plan may work on mobility, strength and lifting technique.

When to seek urgent care

Get urgent help for new leg weakness, numbness around the groin, loss of bladder or bowel control, severe pain after a major injury, fever with feeling unwell, or pain with unexplained weight loss. Seek prompt advice as well if the medicine causes extreme drowsiness, confusion, fainting or breathing difficulty.

Questions for your doctor

Ask: Is this definitely a muscle spasm? Could a nerve, disc or joint be involved? Should I change the medicine or use another approach? Which exercises and activities are appropriate now?

Questions people ask

Should I take an extra dose when the spasm persists?

No. Contact the prescriber first because extra medicine can increase drowsiness, dizziness or other risks.

Does medication failure mean I need a scan?

Not necessarily. The need for imaging depends on the examination, duration, injury history and warning symptoms.
