What if laparoscopic excision of endometriosis does not relieve pelvic pain?

Pelvic pain can persist because tissues are still healing, endometriosis remains in an area that was not removed, or a separate condition is contributing. Pelvic floor muscle spasm, adhesions, bladder or bowel problems, infection, and nerve-related pain may need consideration. The location, timing, menstrual pattern, and triggers help distinguish these possibilities; continued pain alone cannot identify its cause.

Arrange a review with the surgeon or an endometriosis-focused gynaecologist. Bring a symptom diary showing pain location, cycle timing, bowel and bladder symptoms, bleeding, and what makes the pain better or worse. Ask whether examination, imaging, pelvic floor physiotherapy, hormonal treatment, or pain-focused support is suitable. Take prescribed medicines as directed and do not assume that another operation is the next step.

Get urgent medical help for sudden severe pain, fever, fainting, persistent vomiting, heavy bleeding, inability to pass urine, or a swollen and tender abdomen. After surgery, worsening wound redness, drainage, or feeling acutely unwell should be reported promptly. New chest pain or breathlessness needs emergency care.

Ask what the examination suggests, whether the pain could involve the pelvic floor, bladder, bowel, adhesions, or nerves, and whether further tests would change treatment. Discuss symptom control, fertility priorities, hormonal options, and signs that should trigger urgent review.
