What if laparoscopic excision of endometriosis does not relieve pain during sex?

Excision removes visible endometriosis lesions, but pain during penetration can also involve pelvic floor muscle tightening, scar sensitivity, vaginal dryness, infection, bladder or bowel conditions, or pain-processing changes. Recovery itself may temporarily make intercourse uncomfortable. Persistent symptoms do not prove that surgery failed, and pain during sex is a symptom with several possible explanations.

Book a review with the operating gynaecologist or a clinician experienced in endometriosis. Record whether pain is at the entrance or deeper in the pelvis, whether it occurs with certain positions, and whether bleeding, discharge, urinary, bowel, or cycle-related symptoms accompany it. Avoid intercourse that causes significant pain while healing and ask about suitable lubrication, pelvic floor physiotherapy, and other treatment options. Do not self-treat suspected infection.

Seek urgent advice for severe or rapidly increasing pelvic pain, heavy bleeding, fever, faintness, repeated vomiting, a swollen abdomen, or foul-smelling discharge after surgery. Sudden breathlessness or chest pain is an emergency. Contact the surgical team promptly if wounds become increasingly red, swollen, or drain fluid.

Ask where the pain is likely to arise, whether an examination or imaging is appropriate, and how healing appears. Discuss pelvic floor assessment, vaginal dryness, scar care, hormonal treatment, and a safe timeframe for resuming intercourse. Ask which symptoms require same-day help.
