What if inhaler therapy does not relieve shortness of breath?

Persistent breathlessness can happen when the inhaler is difficult to use, the medicine is not suited to the underlying problem, or airway narrowing is more severe than expected. Breathlessness can also arise from infection, anaemia, heart problems, an allergic reaction, or another lung condition. The speed of onset, activity level, chest discomfort, fever, swelling, and response to rest help a clinician judge the situation. Continuing symptoms do not by themselves show that you need more medicine.

Sit upright, stay as calm as possible, and use your prescribed reliever plan exactly as written. Do not borrow another person’s inhaler or repeat doses beyond the instructions. If symptoms settle, arrange a review soon: a clinician can observe your technique, check whether a spacer is appropriate, listen to your chest, and reconsider the diagnosis or treatment. Note the time symptoms began, what you were doing, possible exposures, and how the inhaler affected you.

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, confusion, collapse, severe chest pain, or difficulty speaking because of breathing. Sudden breathlessness after a possible allergic exposure, or breathlessness with facial or throat swelling, also needs emergency care. Use your local emergency number and do not drive yourself if you are seriously unwell.

Ask whether your inhaler technique and action plan are correct, how to recognise an attack, and whether another condition could explain the breathlessness. Confirm exactly when to repeat a dose, when to call for advice, and when emergency help is needed.
