What if GnRH analogue therapy does not relieve pelvic pain?

GnRH analogues lower ovarian hormone stimulation and may be used for hormone-sensitive pelvic conditions. Pain can remain if the underlying condition is not the only cause, if treatment has not had enough time to work, or if pain comes from bowel, bladder, muscles, nerves or infection. Record whether pain follows a cycle, occurs with sex, urination or bowel movements, and whether hot flushes, headaches or mood changes appeared after treatment began.

Do not stop injections or tablets abruptly without advice. Contact your prescriber and describe dose dates, missed treatment, pain location and any side effects. They may reassess the diagnosis, examine you, request imaging or discuss add-back hormones, pain support, another medicine or a procedure. Gentle activity, heat and a clinician-approved pain plan may help while you wait, but worsening symptoms should not be masked indefinitely.

Seek urgent help for sudden severe pain, fainting, fever, persistent vomiting, a rigid or swollen abdomen, heavy vaginal bleeding, or pain with a positive pregnancy test. Get prompt advice for new urinary difficulty, blood in urine or stool, foul discharge, or pain that is escalating quickly. These symptoms can require assessment beyond hormone treatment.

When should this treatment have changed my pain? Could the pain come from another organ or muscle? Do I need examination or imaging? Would add-back treatment or another option be safer for me? How should side effects and missed doses be managed?
