What if an epidural steroid injection does not relieve radiating leg pain?

An injection may not reach the main pain source, or nerve irritation may be continuing because of a disc problem, narrowing around the spine, or another cause. Ongoing symptoms do not by themselves show that the procedure was done incorrectly. Your clinician may review when the pain began, where it travels, numbness, weakness, walking tolerance, and changes since the injection. The pattern of symptoms can help decide whether more imaging, a different injection approach, medication review, or rehabilitation is appropriate.

Contact the clinician who performed the injection or your treating doctor for a planned review. Keep activity gentle and change position regularly; avoid movements that sharply increase leg pain. Follow any prescribed exercises rather than adding strenuous stretching on your own. Note pain location, sleep disruption, leg strength, and any bladder or bowel changes so the appointment is specific. Do not arrange another injection without discussing the response to the first one and the working diagnosis.

Seek urgent medical care for new or worsening leg weakness, numbness around the inner thighs or genitals, loss of bladder or bowel control, or difficulty passing urine. Fever, increasing redness or drainage at the injection site, severe headache that is worse when upright, or rapidly worsening pain also needs prompt assessment. These signs require evaluation rather than waiting for a routine follow-up.
