What if epidural steroid injection does not relieve lower back pain?

An epidural injection is intended to place medicine near irritated spinal nerves. Lower back pain can also arise from muscles, joints, discs, the sacroiliac area, or other tissues that are not the main target of the injection. The timing and pattern of any change matter: note whether leg pain, tingling, or walking tolerance changed even if back pain did not. A limited response does not by itself identify the cause or rule out other treatment options.

Follow the discharge instructions and keep activity within the limits provided by your treating team. Avoid using temporary numbness or reduced pain as a reason to lift heavily. At review, the clinician may reassess strength, sensation, movement, and pain distribution, then recommend physiotherapy, medication review, activity changes, further evaluation, or another procedure when appropriate. Keep a record of new symptoms and any change after the injection.

Seek urgent medical care for new bladder or bowel control problems, numbness around the inner thighs or genital area, progressive leg weakness, severe worsening pain, fever, or a severe headache that is worse when upright after the procedure. Contact the treating service promptly for increasing redness, swelling, drainage, or significant pain at the injection site.

Ask which pain pattern the injection was meant to treat, what the partial or absent response suggests, whether imaging or examination should be revisited, and which activities and exercises are suitable now.
