Why fatigue may continue

Fatigue can have more than one contributor. Anaemia, thyroid problems, infection, pain, sleep disruption, low mood, medication effects and reduced conditioning can all reduce energy. Cancer-related fatigue may also persist while the body recovers, even when treatment has ended. A lack of improvement does not by itself show that rehabilitation has failed or that cancer has returned.

A practical reassessment

Keep a short record of when tiredness is worst, how you sleep, your activity, food and fluids, breathlessness, pain and medicines. Share it with your oncology or rehabilitation team. They may examine you, review medicines, and decide whether blood tests or other checks are appropriate. The programme might need a slower progression, planned rests, energy-conservation techniques, gentle strengthening, sleep support or help with distress. Do not stop prescribed treatment or push through severe exhaustion without advice.

When to seek prompt care

Contact your cancer team promptly for fatigue that is rapidly worsening, limits basic activities, or comes with fever, new pain, bleeding, unexplained weight change or increasing weakness. Seek urgent help for severe breathlessness, chest pain, fainting, confusion, or inability to stay awake. These symptoms need assessment rather than a rehabilitation adjustment alone.

Could anaemia, thyroid disease, infection, sleep problems or a medicine be contributing? Which tests are suitable? Should my exercise plan, rest periods or referral change? What symptoms should lead me to call the team the same day?
